Do you mean embedded images in an HTML email? Outlook usually does not know the name either, especially if the image was pasted from the clipboard.

Take a look at the message with OutlookSpy to see if there are any MAPI properties on the attachment that you can use - select the message, click IMessage button on the OutlookSpy ribbon, go to the GetAttachmentTable tab, double click on the attachment.
