Web Facebook Login “Uncaught ReferenceError: FB is not defined”

Question

I am getting an error when I try to do anything with Facebook login: "Uncaught ReferenceError: FB is not defined"

It seems as if the script is never loading the source from Facebook because window.fbAsyncInit() is never called.
