MVC3 application doesn't show miniprofiler (IIS 7.5 on Windows Server 2008 R2)

Need help with miniprofiler and IIS 7.5 on Windows Server 2008 R2. My MVC3 application works great on localhost but fails to render profile information when deployed on the server. Wondering if I am missing any configuration. Here is my web.config.

I installed latest miniprofiler from nuget. I am running my application in the integrated mode. What am I missing?

Also, while I am referring config file, I am not able to secure access to Elmah (a bonus question) to super admins only.

Is the application pool running in Classic or Integrated mode?

Mini Profiler works out of the box fine when running in Integrated mode. There's supposedly a way to get it to working Classic mode, but the last time I tried (version 1.9) I personally couldn't get it working.
