iOS app Submit response - we are not able to continue because we need additional

I have developed some iOS app but this is the first app which I want to upload to app store. Demo username and password has been given but now new message from Resolution Center on iTunes Connect is -

"Hello, Thank you for your response. We have started the review of your app, but we are not able to continue because we need additional"

Here what is the meaning of 'additional'? Do I have to give all API parameters data ?

I asked this question to to Resolution Center on iTunes Connect. They answered me same statement.

There is a section named "Notes". Please mention additional information there. Such as, how to login, how the app works, how to test the app etc. May be they are not sure about the app flow/features.

Reject issue: “We are not able to continue because we need a demo QR code or AR marker (image) to fully assess your app features.”

Rejected reason: We are not able to continue because we cannot locate the in-app purchases within your app

iOS app got rejected because we must use IAP for payments

Do we need to submit two app to store if we wants to provide in-app feature

Can we still submit iOS app using xcode 4.2?

Do we really need this verification for in-app over iOS 6?

Can we submit iOS app for facebook login review while app is not completed still development?

Can we submit an app to appstore, which have an iOS custom framework in Debug mode?

Can we submit xcode6 beta builds? How to prepare app for iOS8?
