Username and Password not working HTML,FLASK,AND SQLITE3

Question

Im using HTML as front end and Python Flask as backend and for database sqlite3, the code runs but when every time I enter a random value or a password and username that is in the database it seems to give me an (404) error. Both images show my code, Im kind of a beginner to this language.
