login username and password for roundcube

Question

I have recently installed roundcube on my localhost server but the problem is that i can't login into it

When i enter address:

localhost/roundcubemail-0.9.5/

I face to login page with 3 fields (username , password , server ):

(i don't have reputation for posting images!)

What is username and password for it

Is it the one that is in db.inc.php file:

mysql://roundcube:pass@localhost/roundcubemail

or should i register a new user? how?

I have mysql roundcube database but it's tables are completely empty

and what about server field is that

ssl://localhost

or localhost/roundcubemail-0.9.5/

or just localhost

I have searched all of the web but didn't find the answer.
